Shelter step-down → supported housing: the referral workflow

For shelter and street-outreach staff moving someone from emergency shelter toward permanent supported housing. Mai Heaven is a supported-housing resource, not an emergency service.

For an immediate need, call or text 988 for a mental-health crisis, 911 for a medical emergency, or see the numbers for your state. This workflow is for referring someone into supported housing, not for tonight.

The workflow

  1. No wrong door
You do not need to be a government agency to refer. Send what you have and we will review it.
  2. Share the general need
Populations, support level, timeline, and funding source if known — no client identifiers in the form.
  3. Send the referral
De-identified, about a minute, no login. We follow up through a secure channel for the specifics.
  4. One business day to an answer
A clear yes, no, or what-else-we-need — not a week of chasing.
  5. We keep you in the loop
We coordinate with the caseworker who already knows the person, not around them, and we are honest about funding before anyone builds a plan on it.
