The steps
- Send a de-identified referralYou, a family member, or a case manager fills out a short form — about a minute, no login. Don’t include the person’s name or identifying details; we ask only for the general need, the state, and how to reach the referrer.
- We confirm, and a person follows upYou get a confirmation right away, and a real person follows up within one business day — a yes, a no, or exactly what else we need. Anything specific is collected through a secure channel, never plain email.
- The person joins the waiting listThis holds a place on the list and tells us where the need is, which helps us decide where to open first.
- We reach out when a home opensWhen a home is opens near you, we contact you and coordinate admission, transport, and onboarding — with the referring worker, not around them.
Who this is for, and what to have ready
General criteria — every program and state sets its own rules, and a case manager confirms the details. You don’t need all of this to reach out.
Who qualifies
Veterans (discharge status affects which VA programs are open, but an other-than-honorable discharge rarely disqualifies — worth asking); adults with a documented disability, usually with a Medicaid waiver or a place on its waitlist; people managing a behavioral-health or recovery need with a clinician or case manager; and most programs are for people under 50% of area median income, with SSI recipients typically qualifying.
Helpful to have ready
Photo ID; Social Security card if you have it; a Medicaid or insurance card; proof of income (award letter or pay stubs); any current care plan or discharge summary; and for veterans, DD-214 discharge paperwork. Missing paperwork is not a reason to wait — start the conversation without it. Replacing a lost DD-214 or Social Security card is a normal part of the process, and a case manager helps with it.
What we collect, and what we don’t
On the form
The referrer’s name and contact, the general population and need, the state, urgency, and a short non-identifying description. That’s it. No client name, no date of birth, no medical records.
How your information is handled
We won’t contact your employer, landlord, or anyone else without your consent, and you can reach us discreetly — by text, email, or the form, without having to call or speak out loud. We don’t sell or share your information. Nothing clinical should be sent by email — we hold no BAA with an email provider yet, so anything about a diagnosis, medications, or history is collected through a secure channel we agree on with you. Read our resident rights and privacy policy — published before we hold a single licence.
What to expect
A real answer within one business day, and honesty about funding and fit before anyone builds a plan around it. We won’t promise a bed we don’t have, and we won’t disappear — the system escalates internally if a referral goes unanswered past that one-day commitment.
