Placement criteria & accepted populations

One page. Keep it on file, forward it, or print it.

We hold no operating license in any state and cannot accept a placement today. This sheet is what we accept — so you know now, and can refer or join the waiting list. For an immediate need, these are the numbers to call today.

Who we are built for

  • Adults needing daily support who do not require 24-hour nursing or acute medical care
  • Intellectual & developmental disabilities (I/DD)
  • Behavioral-health needs, behaviorally stable
  • People leaving a hospital, psychiatric unit, or treatment
  • Veterans, including hard-to-place
  • Young people ageing out of foster care
  • Re-entry, coordinated with probation/parole
  • Difficult-to-place / refused elsewhere — we want these first

Who we are not the fit for

  • Anyone needing 24-hour skilled nursing or acute medical care
  • A locked or secure unit
  • An emergency shelter or a same-day crisis bed
  • Active psychiatric emergency (call 988) or medical emergency (911)

What a referral needs

Send an incomplete referral rather than wait — we will tell you the one form that is missing. An incomplete referral is still a referral.

How fast you get an answer

One business day for an initial yes, no, or exactly-what-else-we-need. We publish that commitment and escalate internally when it is missed. (This is our commitment on referral review, not a promise of same-day housing — we can’t offer a placement today.)

How to refer

  1. Send the referral and whatever documentation you have — the referral page or the partner portal at /refer.
  2. We review and ask only for the essential missing items.
  3. We answer clearly within one business day: accepted, pending, or not the right fit — with the reason.
  4. When a home opens, we coordinate admission, transport, and onboarding with you.
