Community & nonprofit workers → supported housing: the referral workflow

For nonprofit case managers, veteran-service organizations, faith and community advocates keeping a short list of places that will actually take people. Mai Heaven is a supported-housing resource, not an emergency service.

For an immediate need, call or text 988 for a mental-health crisis, 911 for a medical emergency, or see the numbers for your state. This workflow is for referring someone into supported housing, not for tonight.

The workflow

  1. Anyone can refer
No government affiliation required. If you know someone who needs a place, you can send them to us.
  2. Share the general need
Populations, support level, timeline, funding if known — no identifiers in the form.
  3. Send the referral
De-identified, about a minute, no login.
  4. One business day to an answer
A straight answer, and honesty about whether funding fits before anyone gets attached to a plan.
  5. Warm handoff when a home opens
This joins the waiting list now; we coordinate with you and the person when a home opens nearby.
