If you have received a discharge notice

Contact your state's Long-Term Care Ombudsman straight away. The service is free, independent of any provider, and exists exactly for this. You do not need the provider's permission and you do not have to tell them first.

An involuntary discharge normally requires 30 days written notice, a stated reason, a real destination, and the Ombudsman's details on the notice itself. An appeal filed before the discharge date generally means the person stays while it is decided.

State agencies — licensing and Medicaid waivers

These are the agencies that licence supported homes and administer the Medicaid waivers that pay for them. If you are looking for a placement, or checking whether a provider is properly licensed, start here.

Always verify a licence with the issuing agency, not the provider. Most of these agencies publish inspection reports and enforcement actions online. A provider who will not give you their licence number is telling you something.
