The right door depends on the need. Intellectual/developmental disability or autism: see housing for adults with autism/IDD. Serious mental illness: see long-term housing for adults with SMI. Contact your state Medicaid or developmental-disability agency for an eligibility assessment and to apply for the waiver, or ask a hospital discharge planner or care manager to start it.
