For behavioral health & crisis discharge
For the unit trying to discharge a patient who is stable enough to leave but has no safe, structured place to go. This packet is so you know what we are being built to take, and so the referral is ready the day a home opens.
- Acuity: built for behavioral stability — awake overnight staffing where the plan requires it, behaviour support plan implementation, and de-escalation-trained staff. Not a locked or secure unit.
- Continuity: we implement the existing treatment and behaviour support plan and coordinate with the outpatient prescriber and case manager
- Substance use: a co-occurring substance use history is not a reason we refuse; a 42 CFR Part 2 record needs its own consent and we will tell you which form
- Not a crisis bed today: for an active psychiatric emergency use 988 or the crisis line — this packet is for planned, structured discharge to a community home
Facility Overview
Mai Heaven is a residential care provider building small, permanent homes with real daily support for veterans, adults with disabilities, people leaving hospital or treatment, and young people ageing out of care.
Mission & Care Philosophy
Permanent, not transitional. Small enough that staff know every resident by name. Rights published before we hold a licence, because a provider who only discloses them once you are already living there has the incentive backwards.
Admission Criteria
- Adults needing daily support who do not require 24-hour nursing or acute medical care
- Veterans, including those whose needs never fitted a standard placement
- People leaving hospital, psychiatric care, or substance use treatment
- Young people ageing out of foster care with no adult placement
- Patients described as difficult to place, or refused elsewhere
Levels of Care Provided
Residential habilitation and supported living under an HCBS waiver model. Not a skilled nursing facility, not a locked or secure unit, not an emergency shelter.
Behavioral & Medical Capabilities
Awake overnight staffing where the resident's plan requires it, medication administration, behaviour support plan implementation, and coordination with outside clinical and case management teams.
Crisis Response Process
Incidents requiring a safety plan (allegation of abuse, unauthorised restraint, elopement, death, major medical event, or anything rated serious/critical) are never closed without one on file, and state notification deadlines are tracked automatically.

HIPAA & Compliance Statement
We hold no operating licence and no BAA with an email provider today, so nothing clinical should be sent by email. A secure document exchange is on our roadmap; until then, documents are exchanged by the method we agree with you directly.
