Funding streams at a glance
- Medicaid HCBS Waiver (residential habilitation / supported living) β the core funding for community support for IDD, physical disability, and aging. Started by the person's Medicaid support coordinator. Often has a waitlist β apply early.
- SSI / SSDI β cash benefit for people with disabilities or low income; SSI usually confers Medicaid eligibility, which unlocks the waivers.
- VA housing programs β Grant & Per Diem (transitional beds), HUD-VASH (voucher + VA case management), SSVF (prevention/rapid re-housing). Start with the VAMC homeless coordinator.
- Housing Choice Voucher / Permanent Supportive Housing β rental subsidy via the local Public Housing Authority; homeless placements route through CoC coordinated entry.
- State behavioral-health & opioid-abatement funds β sometimes fund recovery or supportive housing when Medicaid does not.
- Extended foster care & Chafee β for youth aging out of care (transitional living, education/training vouchers).
- Private pay β self- or family-funded, often bridging until a waiver or voucher comes through.

What makes a strong referral
Send what you have β an incomplete referral is still a referral. The fuller picture helps us answer faster:
- Face sheet / current demographics and the current level-of-care determination or assessment
- Current medication list (including PRNs and any controlled substances)
- Behaviour support plan, if one exists
- Guardianship, conservatorship, or power-of-attorney paperwork
- Funding source β HCBS waiver, SSI, VA program, private, or unresolved
- A signed release β records covered by 42 CFR Part 2 (substance use) need their own consent (build one at maiheaven.com/part2-consent)
