For case managers, hospital discharge planners, APS/CPS, MCO care managers, and community agencies.

Mai Heaven operates small, staffed residential supported homes. We want to be a fast, responsive option for your team. Send whatever you have — a full packet is not required to start a review, and we coordinate with clinical providers rather than replacing them.

  • Diagnosis
  • Behavior flags
  • Funding type
  • County
  • Guardian / decision-maker info
  • Preferred language
  • Discharge date (if in hospital)
  • Any documents you already have
