Veteran Supported-Living Referral Checklist
A one-page guide for case managers, discharge planners, and VSOs. Print it, share it, keep it by the phone. Honest about what we can and can't do today.
Compliance
Mai Heaven is an independent referral platform — not a VA, government agency, licensed care facility, or emergency service. When you refer, do not send PHI or SSNs — use initials or a Case ID. Anything clinical is collected later through a secure channel.
Have these ready before you refer
- The general population/need (veteran; and any of I/DD, behavioral health, substance-use recovery)
- The level of support — from light check-ins to awake-overnight supervision
- The county/city where they need to be (not a street address)
- Timeline — planning ahead, or urgent within days
- Likely funding source, if known (see the quick-reference below)
- Preferred language, and whether an interpreter is needed
- A Case ID or the person's initials — never their full name on the form
- Your work email so we can confirm and follow up (no phone needed)
Do NOT put on the form
- Full name, date of birth, SSN, or member/Medicaid ID
- Diagnoses, medications, or clinical records
- Anything you wouldn't want in a plain email — we'll open a secure channel for the rest
Veteran funding quick-reference
| Program | What it helps with |
|---|---|
| VA GPD Grant & Per Diem | Funds transitional supported-housing beds for veterans experiencing homelessness. |
| HUD-VASH | Pairs a housing voucher with VA case management for long-term stability. |
| SSVF | Prevents or rapidly reverses a housing loss (rent/deposit help, case management). |
| VDC Veteran-Directed Care | Gives the veteran a budget to direct their own support services. |
| A&A Aid & Attendance | A pension add-on for veterans who need help with daily activities. |
An other-than-honorable discharge does not automatically disqualify anyone — ask. Full detail: maiheaven.com/how-is-housing-for-veterans-paid.
What happens after you refer
- Within one business day, a real person replies — a yes, a not-now, or exactly what else we need.
- You get a no-login tracking link to see the status anytime.
- This holds the person a place on our list — it is not a placement today. We never promise a bed we don't have.
- If someone needs a bed right now, we'll point you to crisis and placement numbers instead.
No phone number or physical address is ever requested. Questions? Email referrals@maiheaven.com. Mai Heaven is a referral platform, not a licensed care facility.
